Quality is the foundation upon which Mount Carmel Health Partners is built and the cornerstone of our success. The Mount Carmel Health Partners Quality Departments consists of six Quality Specialists and one Quality Manager. Our Quality Specialists work closely with practices to help work their open gap reports, visit their practices, assist with patient outreach via phone calls and letters and distributes monthly newsletters to practices that include quality tips and updates.
The Quality Department works on closing HEDIS quality gaps in care. We do this by:
- Identifying a broad range of health issues.
- Addressing those health issues.
- Working together to close the care gaps.
- Members are seeing considerable improvement on performance measures, which are being tracked closely by payors and employer groups.
A gap in care is defined as the discrepancy between recommended best practices and the care that is provided. This can manifest itself in several different ways, but the most common include individuals missing age-based routine or annual screenings.
